We are aware of the tight schedules you may have so we strongly encourage scheduling your appointments ahead of time. By scheduling your next visit before you leave our office we can reserve the date and time that is most convenient for you.

We understand emergencies will arise and that pain cannot wait. When such an emergency occurs, please call our office immediately. You may be asked to come to the office within 2 hours. We will do an examination, administer localized treatment to ease your pain and schedule an appointment for the appropriate treatment YOU ARE RESPONSIBLE FOR YOUR APPOINTMENTS, WE ARE RESPONSIBLE FOR YOUR CARE. Once you have a scheduled appointment we expect you to honor it.

We respect your time and hope you will respect ours. Last minuet appointment changes are discouraged as openings in the schedule usually difficult to find. There will be a charge of $30.00 fee per half tour of scheduled tine for broken ate. Canceling an appointment with less than 48 hours notice will be considered a broken appointment and charged accordingly. Our patients value their appointments.

We attempt to see our patients within the first ten minutes of their scheduled appointment time. Late arrivals may result in rescheduling your visit A broken appointment fee may be charged if your appointment needs to be rescheduled.

Payment Policy
Patient is responsible for all payments. Insurance may pay all or part of the bill dependent on the particular plan.

Regular Cleanings and/or Check-ups to include New patients & Emergencies

Payment and/or assignment of insurance benefits are due at time of visit and will be paid by cash or credit card prior to treatment. By providing our office manager with a completed dental insurance form or other necessary information we will be able to file with your insurance company for payment immediately.

We are always more than happy to assist you with any questions or issues you may have concerning your insurance company. However, the responsibility of payment lies with the patient. If your insurance company does not pay for the services rendered within 60 days, you will be responsible for the balance.

Third Party Billing
We will help you coordinate your insurance benefits and bill your insurance company if provided with the correct information prior to your appointment. However, the benefits contract is between you and your insurance company. Please be aware that we are not associated with your insurance company nor can we guarantee or be responsible for their payment. You will be responsible for any amounts not paid by your insurance company.

Treatment Plans and Emergencies
We understand that dental treatment expenses can be a burden. We are not set up for payment plans. Any payment arrangement should be made with the office manager before scheduling your treatment. We offer the following payment options:

  • Option A: A 10% discount is given for direct payment in full by cash or check, without 3" { party billing, prior to your scheduled treatment. A 4% discount is given for direct payment in full by Visa, Master Card, or American Express without 3`1 party billing, prior to your scheduled treatment.
  • Option B: Assignment of estimated insurance benefits in coordination with patient co-payment responsibility. The patient responsibility (co-payment) is due prior to your scheduled treatment. The patient will responsible for any amounts not paid by their insurance.
  • Option C: Credit approved financing through various financial institutions, with interest rates starting as low as 0% interest. See Office Manager for more information. Tender Dental care assists patients in applying for CARE CREDIT. A convienent way to pay for costs associated with dental services.

Returned Checks
The patient is responsible for repayment by cash, money order, credit card or certified bank check the amount of any check returned unpaid by your bank. The charge for a returned check is $35.00. Both amounts, if applicable, will be due immediately. The Patients is also responsible for all fees associated with the collection of a returned check.
